The Flagler College Athletic Training staff works under the guidance of six team physicians, listed below. Head athletic trainer Jennifer Rinnert oversees the athletic training staff and services provided to Flagler student-athletes. The professional staff is made up of two assistant athletic trainers, including one specializing in strength and conditioning.

Physical Packets
(Student-athletes under 18 years of age must have a parent or guardian complete and sign all forms.)

All paperwork that must be completed by Flagler College student-athletes can be found below. All of the forms in the packet must be printed out, completed, and given to the sports medicine department prior to or on the day of athlete physicals.

All forms must be completed in their entirety. Forms may not be altered or changed in any way. All forms must be complete prior to a student-athlete participating in any team activity (i.e. conditioning, practice, etc.). If a student-athlete does not have all forms completed, he/she will be held out of all team activities until his/her packet is complete.
